What it does
Many sites end up running separate tools for contact forms, payments, signatures and surveys. This plugin puts those jobs in one place. You build forms by dragging fields into position, then add conditional logic, repeatable fields and calculations when a form needs to respond to what a visitor enters. Ready-made templates, both preset and cloud-based, give you a starting point so you are not building every form from a blank screen.
Beyond standard forms, you can accept one-time and recurring payments through Stripe and PayPal, collect digital signatures and generate PDFs from submissions for your records. Quizzes and polls sit alongside forms in the same builder, with lead capture, live poll results and image support. Spam protection comes from reCAPTCHA, hCaptcha or Turnstile, and styling controls let you match colors and layout to your brand. It suits sites that handle registrations, orders, applications or audience feedback.
Best for
- Businesses collecting payments or subscriptions through site forms
- Agencies handling contracts, consent forms and signed approvals
- Publishers and marketers running quizzes, polls and lead capture

Add a signature field so visitors can sign approvals, contracts and consent forms directly on your site.
PDF generation from submissions
Create PDFs automatically from form entries, which makes it easier to keep records and share submitted information with others.
Conditional logic and calculations
Use repeatable fields, conditional logic and calculations to build forms that change based on visitor input and fit your own workflow.

Questions about Forminator Pro
Can I take recurring payments with this plugin?
Yes. Forminator Pro supports Stripe subscriptions for recurring billing, which works well for memberships and ongoing services. It also handles one-time payments through both Stripe and PayPal, so you can collect money directly within your forms.
How does it keep spam out of my forms?
You can protect forms with reCAPTCHA, hCaptcha or Turnstile. Pick the anti-spam option you prefer and apply it to your forms to filter out automated submissions.
Can visitors sign documents through a form?
Yes. There is an e-signature field you can add to any form, so visitors can sign approvals, contracts or consent forms on your site. Submissions can also be turned into PDFs for record-keeping and sharing.
Does it work for quizzes as well as regular forms?
It does. The same builder creates personality-style quizzes with lead capture, email notifications and social sharing, and polls with image support, live results and custom styling. Quiz lead forms can also connect with Stripe payments.
Is Forminator Pro free?
The code of Forminator Pro is licensed under the GPL, so anyone may use, change and share it. GPL Feast is a paid service: a plan or a single purchase gives you the download and automatic updates. The developer's own support, licence key and online services are not included.
Which version of Forminator Pro is this?
Version 1.58.2, released Oct 12, 2026. New versions appear here when the developer publishes them, and members get them as automatic updates.
How do I update Forminator Pro?
Install the free GPL Feast Updater on your site and connect it with your API key. It installs new versions of Forminator Pro automatically and keeps a backup, so you can roll back.
