What it does
Standard donation forms ask for a name, an email and an amount. Many campaigns need more than that, such as a dedication message, a shirt size, a document or a set of repeated entries. This add-on gives you 14 custom field types to fill those gaps, from simple checkboxes and dropdowns to file uploads, a rich text editor and a Repeater field. You place and arrange them by dragging, so the form stays tidy and easy for donors to follow.
Fields are set per form, so each donation form can carry its own set of questions for a specific campaign or appeal. Answers donors submit are stored with the donation and appear on the individual GiveWP transaction screens, where you can review, edit or export them. The add-on requires the GiveWP plugin. It is documented and built to be extended, which helps developers who want to customize it further for a client or organization.
Best for
- Nonprofits running donation forms with GiveWP
- Fundraisers collecting extra details from donors
- Developers building custom donation forms for clients

Questions about Form Field Manager
Do I need GiveWP to use this?
Yes. This is an add-on for the GiveWP donation plugin, and it works by adding custom fields to the donation forms you build with GiveWP. It does not create forms on its own.
What kinds of fields can I add to a donation form?
There are 14 field types to choose from. They include checkboxes, dropdowns, radio buttons, file uploads, a TinyMCE rich text editor and a Repeater field, which lets donors fill in repeated groups of entries.
Can different donation forms ask different questions?
Yes. Custom fields are configured per form, so each donation form can have its own set of fields. That suits organizations running several campaigns that each need different donor information.
Where do I see the answers donors submit?
Responses are stored with each donation and shown on the individual GiveWP transaction screens. From there you can review and edit the submitted data, and you can also export it for use elsewhere.
Is Form Field Manager free?
The code of Form Field Manager is licensed under the GPL, so anyone may use, change and share it. GPL Feast is a paid service: a plan or a single purchase gives you the download and automatic updates. The developer's own support, licence key and online services are not included.
Which version of Form Field Manager is this?
Version 3.2.1, released Nov 5, 2025. New versions appear here when the developer publishes them, and members get them as automatic updates.
How do I update Form Field Manager?
Install the free GPL Feast Updater on your site and connect it with your API key. It installs new versions of Form Field Manager automatically and keeps a backup, so you can roll back.
What does Form Field Manager need?
Form Field Manager needs GiveWP installed and active.
